2012 Registration Form: This is the registration form that came with your brochure. You can print copies and use them to mail or give them to other people. Please remember to print and complete all information and to sign your form.

Scholarship Application: Please download this form if you are applying for a Scholarship. Scholarships are made by possible by the Ed & Peg Memorial Fund.
Weekend Application: A $35 fee will be added for campers who need to stay on the weekends. The weekends are not a part of our summer camp program and are to be used for the convenience of the parents due to transportation, schedule conflicts and children who are different ages coming to weeks following each other.

Whitewater Rafting Form (Teen Week): This form is for those in Teen Week who wish to add the Whitewater Rafting option. Please fill out this form and submit it with your registration. There is an additional fee of $50 for this option.

Adventure Camp Packing List: This is a Packing List of items for Adventure Camp (Grades 6-7, Grades 8-9).
Adventure Camp (Grades 10-12) Packing List: This is a Packing List of items for the BEYOND weeks of Adventure Camp (Grades 10-12).
Extreme Outback Application: Registrants must download and fill out this application for acceptance into this strenuous week of camp. Please complete the form and mail it to New Life Bible Camp.
Extreme Outback Packing List: This is the packing list for those attending the July 15-20 week of Adventure Camp (Ages 16-20).
Whitewater Rafting Form: This form is for those in Adventure Camp during the weeks of July 1-6, July 8-13, and July 29-Aug 3. Please fill out this form and submit it with your registration. There is NO additional fee for this option. It is included in the week fee.
Retreat Registration Form: This is a registration form for those attending New Life sponsored events. Please make sure all information is filled out.
NewLOS Registration Form: This is a registration form for the New Life Outdoor Education Day.

Statement of Faith: This is the Statement of Faith for New Life Bible Camp. We ask that all Guest Retreats agree to this statement.
